Kelli Vasquez is the Chief Operating Officer and Chief Population Health Officer at Square Care, LLC. Square Care operates a Management Service Organization providing services to a Single TIN Provider Group as well as an IPA. Kelli oversees the operations of the MSO which include managed care strategy and contracting, quality reporting and initiatives, and population health management. Additionally, Kelli oversees the Network Development and Provider Relations department as well as project manages a single EMR/RCM platform implementation across the network.
Prior to joining Square Care, Kelli was the Senior Director of Care Management and Care Coordination at the Suffolk Care Collaborative Performing Provider System. As a member of the senior management team, Kelli was overseeing the Care Management Organization established by the Suffolk Care Collaborative to address the needs of patients throughout the PPS that were not otherwise be managed by existing resources. Kelli was also charged with organizing strategies and improving overall efforts in care coordination throughout the PPS in order to allow patients to effectively navigate the care continuum while reducing fragmented care.
Kelli also held a previous position as the Assistant Director of Care Coordination at an Accountable Care Organization. Through this role, Kelli gained extensive experience in practice transformation and acted as a clinical liaison to the Provider Relations team which included work in areas of quality initiatives and auditing, Patient Centered Medical Home training and consultation, data analytics for risk modeling and development of educational opportunities for staff and physicians. Prior to these leadership roles, Kelli worked in the clinical field of Social Worker as both a medical and behavioral health Social Worker in the areas of hemodialysis, inpatient adult psychiatry, inpatient child and adolescent psychiatry, private therapy, and hospital based discharge planning and utilization management. Kelli recently taught continuing education courses at the Stony Brook School of Social Welfare on Care Coordination and its role in the age of healthcare reform.
Kelli holds a Master’s Degree in Social Work from Fordham University and is a Licensed Clinical Social Worker. Kelli is SIFI certified and is currently enrolled at St. Joseph’s College pursuing a Master’s Degree in Business Administration with a concentration in Healthcare Management.
