Jennifer Torres is the Population Health Project Manager for Square Care, LLC. Jennifer has many years of experience in the health care field. She comes to the team having worked for a large hospital system on Long Island, under their Care Management division within the ACO and IPA. She has over 10 years of professional experience as a case manager for a diverse population which included Traumatic Brain Injury (TBI) patients as well as working with the geriatric population. In her last position as a Care Coordinator, she managed patients via telephone to ensure that patients received all necessary services at home prior to discharge from their hospitalization to prevent re -hospitalizations. Jennifer implemented a variety of resources for her patients which included home care services, physical therapy, occupational therapy, referrals for durable medical equipment (DME) such as a cane, walker, wheelchair, etc.
In August 2017 Jennifer graduated from Fordham University with a Master’s Degree in Social Work and is currently pursing her Licensed Master’s Social Worker (LMSW). Jennifer has a passion for the health care field especially in collaborating with the physicians to provide the best quality of care to their patients. Jennifer will continue to use her knowledge in the health care field and the many years of experience through her journey with the team at Square Care, LLC.
